The National Kidney Foundation of Hawaii has launched a new program for individuals as risk of or in the early stages of kidney disease called KAPP (Kidney Action Plan Program).

Participants will learn valuable self-management skills to delay the progression of kidney disease, diabetes and hypertension.

The objectives of KAPP are to improve self-management of kidney disease, diabetes and hypertension, and to promote a readiness to change lifestyle behaviors in individuals at risk, including those with Stages 2 and 3 kidney disease, through educational intervention. Specifically, the KAPP curriculum focuses on the following core areas of kidney disease self-management:

  • Session I - 8/4/07 - Meet Your Kidneys (About kidneys and their function);
  • Session II 8/18/07 - Create-A-Plate (Nutrition and portion control);
  • Session III 9/1/07 - Cut the Fat (How to reduce the fat in your diet and reading food labels);
  • Session IV - 9/15/07 - Stepping Out (Increasing physical activity and integrating exercise into routine activities of daily living);
  • Session V -9/29/07 - Know Your Numbers (Learning about proper kidney disease, diabetes and hypertension self-management skills to improve glucose control and blood pressure and to reduce and/or prevent associated complications);
  • Session VI -10/15/07 - Talk With Your Doc (Developing strategies in the sharing of information between doctor and patient and tools to help individuals succeed in their quest for a healthier lifestyle and prevent further progression of kidney disease).
